Windows compatibility fixes and hook JSON format corrections.

## Changes

* Fixed Claude Code hook JSON output format (Issue #18)
* Added UTF-8 encoding for git commands (Issue #19)
* Cross-platform `tail_follow()` implementation in status.py
* Hook commands now use `python3` directly

> **Windows Users**: If your system uses `python` instead of `python3`, manually update `.claude/settings.json` to change `python3` to `python`.
