trellis update 现在会把 hash-tracked 的 .trellis/workflow.md 作为完整运行时模板更新。Updater 不再只合并 [workflow-state:*] block，因此 phase heading 和 codex-inline / codex-sub-agent 这类平台路由 marker 会一起刷新。修复 Codex 升级后 hook 脚本已更新、但 workflow 仍停在旧 [Codex] block 的问题。npm install -g @mindfoldhq/trellis@latest
